I'd like to compile a list of those various adaptations listed for classes and PrCs. Those are the variants that are rarely fleshed out in the Adaptation section of classes. Some are described well enough, like the Unarmed Swordsage or the Dead God Ur-Priest, others are simple ideas that need some tweaking to make work, like the Arcane Swordsage and the Stonestorm Striker, and still others are just design ideas with few or no mechanical details, like the Non-Dwarf Deepstone Sentinel. I'd like to not only compile a list of them, like the Alternative Class Features handbook, but also a list of suggested fixes to make them work properly. I don't think it's quite enough to warrant becoming a handbook of its own (most of it could be put in the existing ACFs handbook), but I would like to get the information from the various splatbooks into one place.
Does this sound like a good idea?
Here are the adaptations from Tome of Battle. I've only included the ones with actual mechanical changes described or suggested.
Swordsage (ToB 15-20)

Suggested Changes: If you use RAI that the Swordsage's AC Bonus ability works when unarmored in addition to while in light armor, then no changes are needed. Otherwise, you should make it work while unarmored.
Arcane SwordsageStatus: Needs tweaking and/or clarification
Requirements: No change
Gains: Ability to learn and use arcane spells from the abjuration, evocation, and transmutation schools as maneuvers.
Loses: Light Armor Proficiency, Hit Die reduced to 1d6
Suggested Changes: As a basic idea, this looks like it works out of the box. However, this should have some clarification done to it. Do the maneuver-spells count as extraordinary abilities (the default for maneuvers), spells, or supernatural abilities (the standard for most magical maneuvers). Does the Arcane Swordsage still have access to the normal Swordsage disciplines in addition to the spells? Do they count as having a spell list or as being able to cast spells for the purposes of prestige classes and the like?

Note that this adaptation also allows for other deities and/or races for which a Dexterity-based fighter with some divine powers makes sense to take the class. Human worshippers of Pelor was just the only example of such.

Whether this class should be available for non-Raptorans is not addressed. If you make this into a prestige class focused on an element other than air, you should probably look for another, more appropriate race. Don't forget to modify the Flight ability requirement and the Instant Supplicatication, Servant Supplication, Assumptive Supplication, and Skypledge abilities, too.
Adaptation
As written, the prestige class reflects an important event in raptoran history: the Pact with the air elementals. It can also serve as a model for any spellcasting prestige class that has pledged itself to an extraplanar power. If you alter the class in this way, you can leave the spellpool class feature intact and develop new abilities to replace the beckon and cyclonic doom class features.

Rather than stonedeath strike as a 5th-level ability, perhaps a poison thorn assassin can exude its own poison periodically for a particularly deadly attack.

The Jaunter prestige class from the Expedition to the Demonweb Pits module has an adaptation for allowing it to advance spellcasting 5/5.
Basically you give up the Travel Power(su) (daily uses of benign transposition, baleful transposition, teleport, and plane shift) in exchange for:

Use the standard Base Attack Bonus for the spellcaster in question (that is, average for clerics or poor for wizards).
This version of the prestige class should have one good save (either Reflex or Will), and the others should be poor.
The prestige class can grant a full spellcasting progression if you simply remove the travel power ability (and all its subordinate powers) and add the spells on the following table to the jaunter’s spell list.
