There is no source for that, but generally, it means you can't Shapechange into the Tarrasque (since it's noted to be one of a kind) or into gods.
Technically, in the Forgotten Realms, there were originally 5, 2 of which are dead, so you could argue...

One per "continent". The one (of two) on the main FR landmass was down in chult, and may or may not be dead, depending on which religious group you believe.

Q116 Changing the Ninja from Sudden Strike to Sneak Attack, giving a free [Ki] feat every 4 levels and making Ki uses per encounter(instead of per/day).

How significantly would this bump it up in tiers? Would this bring it to 4?

you might be able to use the swift ambusher feat for that, but it is a bit DM dependent.

Yea the reach is definitely the least clear.  The logic I applied was as follows: A small longspear is an appropriately sized longspear for a small race and would give a small race reach.  The SRD makes it clear that reach for a small race is the same as reach for a medium race (10 feet), so a small longspear should still grant reach to a medium sized race.

Fusion with one and a way to make that permanent. (You could use Astral Seed trickery to come back as the "fused being", for example.)

Thing is, that Telepathy only works on others of it's kind, so if you're planning to use that for Mindsight, it won't be as good as you probably think.

RAW reading of the feat, I'm not sure if this matters.

 One of my players is getting uppity about low loot count so I'm making an involentary shape shifting weapon and would like something more epic than a D% random weapon table from some book.
A player once asked me if there was any way to make a Tarrasque more evil... 3 sessions later he was stoned with D20s as the PC led an exidus out of the path of a Dire Tarrasque of Legendary Wonder.
