That's evil. Socialized Healthcare at its finest :P I like :)
You should also infect like 1 in 10 or 20 with an infectious disease, to keep a steady stream of business.  :smirk

They don't/haven't, was the point. 3.5 is as dead as people not liking nice tits.

Sometimes, their tits (3.5) get enhancements (houserules), but that doesn't mean people don't like nice tits.

Though sometimes, the surgeon (DM) botches them pretty bad...

No, you don't do that at all.  What kind of evil genius are you?

What you do is keep this up for a couple of decades until nobody has much of an immune system, any items of remove disease are long since gone (buy them up on the secondary market yourself if you have to), and most clerics or druids have completely forgotten they could prepare the spell, since there was never much of a need for it.

Meanwhile, with all the disease samples you've taken over the years from the most disease ridden areas of every major city on the continent, you selectively breed several super diseases, preferably ones with high wis drain.  Then, one day you abruptly close up shop everywhere, simultaneously, have an artificer retain essence on your disease curing and agony draining equipment (which you were making with your proceeds - exponential growth), and release the plague.  Actually, release the plague a week or so earlier, have it display symptoms about two weeks later and be infectious for the first two weeks.  Oh, and make it resistant to magic, too.  And have a false flag attack on own hospitals (ethergauts are rather unpopular), so as to deflect suspicion.

You now have a stockpile of craftable xp and gp that exceeds that of any dragon's hoard, the world's in shambles as the plagues you had stored up are now infecting the completely nonresistant populace, the high level characters will be blaming this on the ethergauts or trying to stop the spread of the disease, and the world will turn to you as a savior.

If your true nature is revealed, you have a lot of magical cutlery and the only stockpile of disease cures - a huge advantage in taking over the world by force.

If it isn't, you still have a lot of magical cutlery, the only stockpile of disease cures, a population that will welcome you taking over and stopping the plague, and your major opponents have been killed off or otherwise incapacitated.

If you want, you can go into the drug trade as well.

Q 224- If Small and Medium creatures threaten 2 squares (10 feet) with a reach weapon, how much space does a Large creature (or one with Powerful Build) threaten?

A224: Powerful Build does not increase reach.  If you have Powerful Build, you have the same reach as any other medium creature.

For large creatures, I don't recall if they threaten from 15' out to 20', or if it's 10' to 20'.  I believe the former.

A large creature (normally) threatens 10 feet. Reach weapons double your natural reach. So a large creature with a reach weapon threatens 20 feet.

EDIT: Question 222~ What is the deadly traveller build? and what's it do?
A 222: The bare bones: used Lightning Leap (Complete Mage) and sculpt spell to move around and deal damage to everything nearby. I fiddled with the build (it wasn't mine originally) a bit later on and tried combining it with eldritch theurge and even arcane archer for better range.

Whiskey Tango Foxtrot?  Why does this work and not sculpting Whirling Blade?

Q 225: "Two-Weapon Fighting"- "If you wield a second weapon in your off hand, you can get one extra attack per round with that weapon. You suffer a -6 penalty with your regular attack or attacks with your primary hand and a -10 penalty to the attack with your off hand when you fight this way. You can reduce these penalties in two ways."

Am I interpreting this correctly? The "when you fight this way" refers directly to when a character is holding a weapon in each hand, right? The TWF penalty doesn't go away unless their off-hand is empty, and applies even when the character makes a single attack as a standard action if they are holding a weapon in the off-hand,  correct?

I believe the FAQ covered this, but unless you actually make an attack with your off-hand weapon, the penalty doesn't apply. The example is set by shields - you can bash someone with them, but unless you do that and swing a weapon, you don't apply TWF penalties. Same if you swing a sword with a shield in your off-hand.

Right. "Fight this way" means use both weapons in a way to get an extra attack per round. I think technically you could even make iterative attacks with two weapons and as long as you don't claim an extra attack from TWFing, you don't take any penalties for doing so. You can certainly make an attack of opportunity with any weapon you have readied.

Q226: Is there a feat that grants paladins +3 cleric levels to their turn undead attempts??
A player once asked me if there was any way to make a Tarrasque more evil... 3 sessions later he was stoned with D20s as the PC led an exidus out of the path of a Dire Tarrasque of Legendary Wonder.

Perfect! I'd like to make one, seems simple I'd want it better! Isn't there the stormcaller prestige class dealing with lightning?

Apply the fiendish template somehow?

Failing that, get the Drow ACF (or feat, I forget) that adds both the chaos and evil descriptor to your spells?
