Q100: I need to enlarge my Fighter, or at least make him be considered larger than normal (he's human) for purposes of Bull Rush/Trip/etc. attempts. What are the best ways to do this, preferrably via feats/magic items and without dipping? (DM specifically excluded Jotunbrud)

Q101: If you provoke attacks of opportunity as part of a charge and you have Robilar's/Karmic Strike, can you Bull Rush your opponent with the attack of opportunity your foe provoked?  :smirk

A100 Potion of Enlarge Person? Wand of Enlarge Person for your sor/wiz? There's also a belt in MIC (1/day) for 3000 gp.

A100 1 level dip into psychic warrior for expansion?

A 100

If custom items are allowed, another option is to get a use-activated or command word item of Enlarge Person for 12k or 10.8k, respectively.

A101 - According to the rules, this is acceptable. Bull rush is fine as part of a charge, and they did provoke the AoO.

Q102 - If I cast Mantle of the Icy Soul [Spell Compendium] on myself to get the Cold Subtype for Immunity to Cold, and I already have the 'natural/granted' Fire Subtype for Immunity to Fire from using Mantle of the Fiery Spirit [Sandstorm]. Does one cancel the other out, or do they stack?

Mantle of the Icy Soul was nerfed in SpC. Not only is it not permanent anymore (1 hour/lvl duration now), it explicitly states that the types cancel each other out.

Q 103: If I'm dropping onto someone with a 10' reach weapon, I don't trigger an attack of oppertunity until I've gacked him right?
A player once asked me if there was any way to make a Tarrasque more evil... 3 sessions later he was stoned with D20s as the PC led an exidus out of the path of a Dire Tarrasque of Legendary Wonder.

A103 - Yes you do, unless you catch the person flat-footed. Movement on any axis is sufficient to trigger an AoO.

Q 98: Since I've seen people talk about using limited wish's spell mimicry use for the power psychic reformation, would a recaster or somesuch class able to grab spells off of any class list thus be able to take psionic powers that way, like fusion or psychic reformation?
A 98: No, psychic powers are not spells. Reproducing Psychic Reformation would fall under this line, I think:
Quote
Produce any other effect whose power level is in line with the above effects, such as a single creature automatically hitting on its next attack or taking a -7 penalty on its next saving throw.
A 4th level power is "in line" with the power of a 4th level spell.

A103: Assuming you have greater reach than your enemy, then you won't trigger an AoO until after you have attacked. This is assuming you're falling straight down onto the enemy. If you just fall past the enemy, and never enter the other guy's reach, you could even avoid drawing an AoO at all.

A82:

No, there are none to my knowledge (and i like to think it is pretty extensive).

But, feat rogue variant from unearthed arcanna, or the divine oracle prestige class from complete divine could grant you something similar.

Q105: Are there any templates that increase flyspeed, fly manuverablity or jump checks?

A105: Ghost1 increases manuveribility to perfect, or gives you 30' perfect if you had less speed.  Not sure that's what you want, though, given the baggage it gives...

Most templates that grant a fly speed allow you to choose the better of the granted speed/maneuverability or your base values.

A 105: I have a bunch of methods listed in my "lists of stuff" thread. The Dark template (ToM) adds 10' to all modes. The Shadow Creature template (LoM) multiplies by 1.5.

Q83: Are there any good ways (other than Soul Trap / Bind) to keep something dead permanently, no matter what?  There are a lot of spells that make resurrection difficult or allow only wish/miracle to work, but is there a way to kill someone beyond the ability of even wish to bring them back?
A83: Killing someone on Malbolge prevents them from being raised or resurrected, but there is no clause for wish.
