You can have the centipede be fiendish... also the Phaerlin Giant from Monsters of Faerun is Huge and CR3, but I don't know if you could really "ride" it unless it's a piggyback ride.

I don't see why not, Incubi ride Succubi all the time.

There's also a feat, I believe in a Dragonlance Monster Manual, that allows mounts to carry someone their size.

Don't quote me on it, but killing bigger gods would be my guess.

Typically that requires something out of the ordinary though, like a major artifact or two.

The RAW way is... actually explicitly DM fiat.  There's a section in the back of Deities and Demigods that talks about potential ways, but it's all left completely in the hands of the DM to decide on the mechanics.

Q116:  OK so a 30 Headed Hydra with 49 HD has a bunch of epic feats...  Which ones are actually worth taking?  I've got the Mage Slayer line, Robilar's Gambit, etc.  Items are pretty scarce / limited, so would Blinding Speed or Epic Speed actually be worthwhile?

Unholy water is about the only thing I can think of. And it's a stretch at best.

Maybe there's a monster out there with a SU or SP ability that deals profane damage, but typically you don't see anything like that. Similarly, Flame Strike is the only spell I can remember that deals holy/divine damage and not positive energy damage off-hand. As a general rule, that's an exceedingly rare damage form.

Never on racial HD, once they hit 21 total HD with class levels.

Arms & Equipment Guide had some guidelines for feats in items.
I am constantly amazed by how many DM's ban Tomb of Battle.  The book doesn't even exist!
