What are the Nastiest, Most Disgusting Creatures in D&D

What are the worst kinds of filthy, perverted bits of flotsam available in 3.X D&D?

It's for a character concept, so it totally counts for this forum!

That was somewhat misleading on my part. Think of it as self-confidence, self-image, the ego, and other such things, not people skills. Someone with a low Charisma who goes insane is probably suffering crippling self-doubt or has a skewed perception of him/herself (or, given the power of the entities in the Cthulhu mythos, an accurate perception of his or her place in the universe).

Even without the sadism it could be appealing to some.  When your Str score is anywhere from 3-6x your Dex score you tend to break things by mistake with inadvertent rough handling.  Breaking people by mistake really kills the mood, you know?

The designers seemed to have an unhealthy obsession with body-part-themed undead. There is the brain in a jar, the jellyfish that plucks out eyeballs, the giants head that walks upside-down with its hair and mind controls people, that creature that is made of nothing but spider legs...
Most of those creatures are dumb, anyways.

Sounds a little OP.

The monk is clearly the best class, no need to optimize here. What you are doing is overkill.

It's like people who have no idea what a turn signal is. They ruin it for everyone else.
When another driver brandishes a holy symbol and begins glowing with divine light, seek cover or get spattered with zombie brains. I do not see what is so complicated about this.
