Q 97 Are there any dimunitive or smaller races that are legal cohorts (eg have a printed LA, or something of that sort). Fey are preferred, high mental stats a plus.

A.99: They would give feats and skill levels, increase BAB and Saves, and possibly ability bonuses though, I'm not sure on that.

Does a warlock get his strength bonus to attack or damage with a eldritch glaive? I think it's str to attack but none to damage but I'm not sure

Q 102:  A friend of mine is playing a Warblade/Eternal Blade 2-Handed Wielding semi-optimized Snow Elf.  Our DM allows us to submit a short list of magic item requests.  2-3 minor, 2-3 moderate, 2-3 major.  He will distribute the items over the course of the campaign.  I'm looking for recommendations for best magic item choices for the 3 categories, other than +X to Str items.

Did you check the Bunko's Bargain Basement thread? Also, this seems like it could benefit from its own thread, rather than just a post in here. I think Steadfast Boots, an Anklet of Translocation, and a Healing Belt and Belt of Battle are good items for that kind of character, though. Of course, he'll want a way to fly, as well.

Q104: Does using these feats together constitute a valid PP recharge setup?

Body Fuel [Psionic]
You can expand your power point total at the expense of your health.
Benefit: You can recover 2 power points by taking 1 point of ability burn damage (see page 67) to each of your three ability scores: Strength, Dexterity, and Constitution. You can recover additional power points for a proportional cost; for example, you could choose to recover 6 power points by taking 3 points of ability burn damage to Strength, Dexterity, and Constitution. These recovered points are added to your power point reserve as if you had gained them by resting overnight.
Special: Only living creatures can use this feat. You can take advantage of this feat only while in your own body (if you are under the effect of mind switch or metamorph, for example, you gain no benefit).

Psychic Bastion [Psionic] (Hyperconscious p.44-45)
You fortify yourself against ability depletion.
Benefit: While you maintain your psionic focus, you gain ability depletion resistance 3 against any attack that depletes an ability score, including poison, ability drain, ability damage, or enhancement penalties to ability scores. For instance, if some power, spell, effect, or poison deals you 4 points of temporary ability damage, you actually take just 1 point of temporary ability damage. The ability depletion resistance works on a per-attack basis.

I'd say yes, but then you can't benefit from Body Fuel (and likely anything else that causes ability burn). Read it again. It says you gain the benefits when you take the damage. If you don't take the damage, you don't get the benefits.
