Mostly-Pointless Accuracy Optimization

Take an Avenger of any race with a STR bonus; give them 20 STR via Demigod.
Give him Warrior of the Wild and the 3 powerswap feats.
PMC into Ranger, snag Twin Strike.
Take a +6 Double Sword.
Take Weapon Expertise for Light Blades and Heavy Blades.
Take Combat Advantage.

Total Attack Bonus of +42 at level 30.
Average AC for a level 30 monster is 44; you hit on a 2.

Declare an average L30 enemy your Oath of Enmity.
Smack it with Twin Strike.

Probability of 1 hand hitting:
(1/20 chance of failure)^2 = 1/400 chance to not hit
Probability of either hand hitting:
(1/400 chance of failure)^2 = 1/16000 chance to not hit = 99.999375% chance to hit at least once at will against your target.

Not really any point, though, unless you just really dislike missing.

Anyone got the numbers for Tiamat on hand?

You could just skip PMC and go with Half Elf with Versatile Master (PHB2), that you allow you to get something actually useful as a PP :)

I considered that, but Half-Elves are CON/CHA, so no STR bonus.

Yea but it would allow you to grab something that give +hit on AP (or a reroll, yea that would be nice :P). Granted it's only once per encounter, but we're talking about something quite crazy :)

Kensei makes up for the missing Str and adds more damage at 16, overbalancing the PMC in the long run for non-hitting factors.

Weapon Expertise does not stack with itself, so the accuracy goes down slightly - this was errata'd pretty quickly.

The Swordmage has a Level 16 encounter Utility that lets you roll all attacks twice until the beginning of your next round. It won't help your current build but might lead to another...

If you are assuming combat advantage with a doublesword, you probably want the Nimble Blade feat as well.

Epic Destiny, right there in the Player's Handbook.

Newb question but what/where is Demigod?
In 3rd edition, a Demigod was well-nigh unbeatable; slinging 3 or more 9th level spells, as Spell-likes. Piles of extra abilities too.
Almost all published monsters, were weaker than a run-of-the-mill Demigod.
PCs probably could not beat a Demigod ever, without CO-board help. Available in the on-line SRD.

In 4th edition, a Demigod is a smidge better than the average 21st to 30th level PC, with any one other Epic Destiny.
The Capstone ability, is about as strong as taking four to eleven 1st level spells from 3e, and being able to cast them at-will.
Most of the published Epic tier Monsters, can kill an average Demigod.
Contra, the average party of Epic tier PCs, can kill those monsters. Available as per Agita above.

Same words, different game.
