If I may offer a very atypical alternative:

Elan Psion. Take the Psicrystal. Manifest "Share Pain" on it and share "Vigor" with it if the fight actually looks hard.
18 Int, 18 Con, 16 dex, 4 Str...12 wis, 9 Cha(modified for race)

"Entagling Ectoplasm" as a cheap debuff. Ego Whip against non-sentient, non-mindless foes.

Probably Telepath for "Psionic Sugestion": target me, which is not only not against their nature but is a very reasonable suggestion so they should get a penalty. And later, "Hostile Empathic Transfer" which gives you a very real benefit to being in melee and allows you to send back all the pain they gave you.
Take the Improved Elan Resilience(CPsi) feat to as an immediate action spend power points to prevent damage at a rate of 4 damage/PP spent(instead of 2/PP as the racial ability).
Also Psionic Body and have almost exclusively psionic feats.
Actually, I played a warforged psion starting at low levels with the Adamantine Body feat, and he could have easily been the "tank" for the party, as he had the best AC and hit points (after using Vigor and/or Share Pain). That's an interesting idea. I also used to coat myself, my psicrystal, and my weapons in contact poison all the time via Psionic Minor Creation. Overall, it was a fun and strong character. From levels 3-8, non-casting melee can actually do fairly well. I'll second the suggestion for a spiked chain tripper. I'd go either goliath barbarian with the substitution level that makes you large when you rage, or maybe some kind of psychic warrior build. A couple of hexblade levels for a +3 to will saves wouldn't be a bad idea, either. Or maybe even a 2 level dip into monk, if you want to try grappling some.

I think Half-orc paragon combined with monk and hexblade should be fun. You can be a feral monk with an attitude problem. You'd have great saves, a +4 racial bonus to Str, and Rage.

Something like this (rearrange the order to taste):
HoP3/Monk2/Ftr1/Hexblade2

Feats: EWP: Spiked Chain (1), Extra Rage (3), Combat Expertise (m1), Improved Trip (m2), Improved Grapple (Ftr1), OPEN feat

Passive Way (Unearthed Arcana alternative monk fighting style) [spoiler]
The Passive Way focuses on making your opponent overreach himself or underestimate your skill.
 
1st-Level Skill Bonus:   Bluff.      
1st-Level Feat:   Combat Expertise.      
2nd-Level Feat:   Improved Trip.      
6th-Level Feat:   Improved Feint.      
6th-Level Bonus Ability:   You gain a +4 bonus on Strength checks made to trip an opponent who is denied his Dexterity bonus to Armor Class.      
Prerequisites:   Bluff 4 ranks, Sense Motive 4 ranks, Skill Focus (Bluff).    [/spoiler]

Base saves would be F+8, R+4, W+7
Sure, there's that, but when you talk about half-orc tanks you have to bring up the Menacer.
