Jermaline (MM2) makes a nice race for both ability contests and hide contests:

Ability: Venerable jermaline -> +9 racial to Wis. Add Precocious Apprentice (Owl's wisdom) for a max 31 wis at first level

Hide:
 Beguiler 1, Dex 24 (26 w/ compression), Int 15, cha 5, rest min
 Feat: Precocious Apprentice (invisibility)
 Skills: maxed hide, maxed UMD, maxed UPD
 Items: Scroll (Camouflage) 25 po, Psionic stone (compression) 25 po, Masterwork skill tool
 Result: 76 -> +12 size (diminutive), +8 Dex, +10 circumstance (camouflage), +40 untiped (invisibility), +2 MW tool, +4 ranks. 96 when taking 20
Without external help.

EDIT: It could just replace Whisper Gnome as the race for Skydragon Erudite build for a +2 to hide and independence from UMD/UPD.

Venerable Elf Wizard: lowest HP.

Con: 3. HP: 1

EDIT: Middle-aged Kobold Barbarian

Frail, Quick, Pathetic.

CON: 3

Draconic Rite of Passage... that's all I can come up with, and that's 5 HP. Not exactly as low as I'd hoped.
