I'd recommend changing the Factotum's Skill list area to just say "Everything."  Much simpler and gets the point across better.

There might be room for the phrase "Int to damn near everything" in a description for it too.

You forgot the OA shaman, who is either very high tier 2 or low tier 1. Between an animal companion, turning attempts and spells to mimic wild shaping he is pretty darned powerful.
Monks are pretty much the best designed class ever.

I haven't forgotten this. I still don't know the source for Magewright. Psychic Rogue was already in there.

I'm ashamed that I did forget OA shaman even though its probably my favorite class ever. Its added.

Nonstandard casting (its not actually druid list like spirit shaman) :( means low t2. It has an AC but can't dmm as well as a mystic

I just noticed that the Nightstalker (Races of Ansalon p.153) is missing. A quick glance at it makes me think it's Tier 3.

Seems like a beefed up adept with built in leadership, Tier 3 seems appropriate.

They greatly benefit from expanding it, like any sane Beguilers or Dread Necromancers (or Warmages) that takes Arcane Disciple or similars because their base list is so goddamn restrictive.

Did I read Nightstalker's ghost cohorts correctly, in that their level is based on your HD (not your class level), and that they are limited by HD, not ECL? Would that mean that you can get cohorts of higher ECL than you (considering only Ghost's +5 LA and not the stupid template stacking that gets applied to Effigies as well)?

Pretty much, I thought their spell list was about on par with the adepts, then adding in the other abilities, and ghost should raise them a tier.

"than your character level" so no. Basically its up a feat compared to leadership, unless you take multiple ghosts in which case it has more HD restrictions. Is 3x15HD ghost cohorts > 1 19HD nonghost cohort? Sure you'd be down two feats but I don't know if that really makes it t3 material. Remember that I did stick it high in t4...

Eh? I thought that Leadership capped at 2 levels beneath you, and at 17th level regardless without Epic Leadership. I know there's a feat that gets you up to your level -1 instead of -2 (Close Cohort or something), but it's still limited by the chart's max level (17th at 25 Leadership score).

The biggest difference between T3 and T4, if I understand the tiers correctly, is versatility. Being able to do whatever you do (even with 6th level spells from an unimpressive list) and whatever your 1-3 incorporeal buddies are doing has to count for something. If it's one ghost, you have 17HD (so 9th level spells), and if it's 3, you have 15 HD each, so, say, one with 8th level spells, one doing whatever Telekinesis abuse TML can think up with 15 levels, and maybe one getting 9th level spells anyways via Ur-Priest or something. Having 45 extra class levels to play with has to count for a hell of a lot.

Hate to get picky with grammar, but it bothers me:

1) Tier 3s are missing a "Do."

2) Tier 2s need an apostrphe in "it's" since it is a contraction of "it is."
"Its" is the possessive form

When this is done, it will be the most valuable resource on these boards. I can't wait!
The answer to everything:

I don't know if the designers meant you to take Skill Focus for every feat.
Sounds a little OP.

The monk is clearly the best class, no need to optimize here. What you are doing is overkill.

It's like people who have no idea what a turn signal is. They ruin it for everyone else.
When another driver brandishes a holy symbol and begins glowing with divine light, seek cover or get spattered with zombie brains. I do not see what is so complicated about this.
